    How to add custom questions to the surveys

    Crisalix built-in communication and marketing tool allows you to gather even more specific insights from your patients and gain a deeper understanding of their experience and how they perceive your service by means of surveys. For that, patients will receive in their inboxes some post-consultation and post-procedure surveys. You can tailor these surveys with two custom questions that will give you further feedback on specific topics relevant to you and your practice. 
     

    1. From the main menu of your personal home page, select Patient App.
    2. From the tabs available, select Surveys. 

      If your surveys are disabled, select Patient App > Settings to enable them.

       
    3. At the right, select + Add custom question. 
    4. Select the type of question you want to add:
      1. Single choice question: patients can only select one answer from the options you provide.
      2. Multiple choice question: patients can select two or more answers from the options you provide.
      3. Text field question: patients can write their own answers to your specific question.
    5. Enter your question in the field provided for it.

      You can use the Preview Survey functionality to overview all the questions available in the survey. Observe that this preview functionality is interactive, and some questions will not be shown until an answer is selected. 

       
    6. If applicable, add as many answers (Option 1, Option 2, etc.) as you want, with a maximum of six. 
    7. Optionally, you can add an “Other” answer, where patients can enter a text with their own answer.
    8. Select Save to save your changes.

    You can repeat this process for the post-consultation and/or post-procedure surveys. Your custom questions will be directly accessible from the Surveys tab. You can delete, modify or edit them at any time. 

    Remember to check the specific regulations of your country or specific for this field.
